Job Description

Position Summary:
We have an exciting opportunity to join our team as a primary care Social Worker.

The Primary Care Social Worker treats patients as part of the Family Health Centers at Sunset Park Family Health Center (Women's Health). This position's goal is to expand the delivery of behavioral health services, including the implementation of an evidence-based prenatal/postpartum depression prevention initiative (ROSE). The Primary Care Social Worker will be based at 5610 2nd Avenue, will have close collaboration with the primary care staff, and will be an integral member of the patient care team. The main functions will be to track women who are at risk of developing mental health conditions, identify pregnant patients at risk for postpartum depression (PPD), provide a manualized brief group- or individual-based PPD prevention intervention, provide individual psychotherapy, and refer patients out to more intensive mental health of substance use services when necessary.

Job Responsibilities:

  • Receive medical patients from the provider when they screen positive for mental health conditions ("warm hand-off" referral);
  • Conduct biopsychosocial assessments focusing on mental health and substance use evaluation;
  • Identify women at risk of developing postpartum depression;
  • Provide a brief, 5-session, manualized, group-based postpartum depression prevention intervention, using an evidence based treatment.;
  • Conduct psychotherapy with patients;
  • Develop and carry out a treatment plan in EPIC based on the presenting behavioral health issues;
  • Document psychotherapy visits/ progress notes in EPIC within 72 hours;
  • Provide confirmation calls and outreach calls as appropriate;
  • Provide clinical feedback on patient progress to the referring provider;
  • Work with care team and site leadership to assess workflow for screenings and referrals and adapt accordingly;
  • Collaborate with ROSE implementation team to assess patient receptivity to program and assist in making adaptations when appropriate;
  • Assist in the preparation of reports to funders.
  • Participate in the site's team meetings and daily huddles, with a focus on care coordination, educating medical staff about the role of the social worker and the importance of mental health assessment and treatment.

Minimum Qualifications:

  • LCSW/LMSW in Social Work, license current and in good standing
  • Bilingual English/Spanish
