Responsibilities:

•    Participates and supports the health team in direct patient care. Performs duties involving direct patient care as assigned by the physician within the framework of the New York State Nurse Practice Act and AdvantageCare Physicians' policies and procedures.
•    Identifies physical, behavioral, and emotional characteristics typical for the age group of the patient receiving care. Modifies approaches based on patient age-specific needs and responses to treatment.
•    Uses age appropriate communication techniques.
•    Triages and assesses patients according to nursing process, by telephone and in person and documents appropriately in patients Electronic Health Record (EHR).
•    Monitors office flow to assure efficiency, assigns daily lunch schedule to staff to avoid/minimize disruptions to patient flow.
•    Demonstrates excellent clinical skills. Is a role model and resource for the clinical staff.
•    Delegates tasks appropriately according to scope of licensure and follows up on any delegated tasks.
•    Anticipates, prepares and assists with various medical procedures, as required, e.g., minor office procedures and biopsies consistent with the NY State Nursing Guide to Practice.
•    Prioritizes patient care by implementing physician orders and nursing process. Acts as a patient advocate and upholds patient rights.
•    Reviews and updates medication list, documents known allergies in EPIC.
•    Provides medication verification on recently hospitalized patients in EPIC and ensures recent labs have been scanned. Collates all information as required for visit.
•    Obtains prior authorizations for medications and Durable Medical Equipment (DME), notifies patient and physician, documents in EPIC.
•    Acts rapidly and calmly during emergencies, initiates cardiopulmonary resuscitation (CPR) and provides the necessary items called for by the team.
•    Collects and accurately labels specimens.  Prepares specimens and required paperwork for collection by affiliated laboratories.
•    Retrieves lab, pathology and diagnostic data from the outside laboratories and collates prior to visit.
•    Ensures that all pertinent equipment is sterilized according to schedule based on the purpose for and use of the equipment.  Notifies appropriate personnel promptly regarding any malfunction in equipment.
•    Prepares the patients for physical examination, chaperones the patient during examination, as needed.
•    Assures all patients are correctly identified prior to providing and/or documenting care. Provides patient education as directed by provider. Documents in EPIC appropriately.
•    Keeps examination rooms neat and clean. Restocks exam/treatment rooms and monitors inventories of non-biological stock. Assures minimum levels are maintained in all exam rooms. Monitors appropriate use of all supplies.
•    Ensures medication and treatment items are not expired. Maintains orderly sample inventory and log if required.
•    Checks and maintains logs for emergency equipment according to AdvantageCare Physicians’' policy. Reports any malfunctions promptly to appropriate personnel.
•    Meets regulatory and annual health assessment requirements. Maintains current Basic Life Support (BLS) certification.
•    Appropriately adheres to universal safety precautions when administering medications and disposal of medical waste.
•    Demonstrates a professional, courteous, welcoming, tactful, patient, and respectful attitude in dealing with patients, families and significant others, at all times. Proactively seeks to minimize any patient concerns/issues by addressing immediately or by referring to appropriate administrative team member.
•    Answers telephones in a professional manner utilizing excellent customer service skills exhibited by courtesy, cooperative spirit, and tact.  Utilizes standard AdvantageCare Physicians greeting at all times when answering the telephone
•    Demonstrates proficiency with computer systems (e.g., electronic practice management/electronic health record) within 3 months of training.  Completes all tasks on the electronic system that are required for the job function consistently, accurately, and in a timely manner.
•    Takes complete and accurate messages from individuals contacting the medical facility by telephone, including messages related to patient needs intended for providers and clinical staff.  Delivers messages to the intended recipient in a timely manner.
•    Checks (EPIC) email correspondence.
•    Coordinates with support staff to remind patients of appointments and collect appropriate information prior to appointments.
•    Monitors patient flow in exam rooms and waiting room(s).  Proactively resolves patient flow issues related to the clinical visit process. Keeps patients informed of any delays, in the waiting room and the exam room.
•    When bilingual, performs basic conversational interpretation for providers and staff, within the limits of his/her capabilities.  Informs the provider/staff of any word(s) or term(s) s/he is incapable of interpreting.
•    Is punctual and maintains a satisfactory attendance record.
•    Demonstrates professionalism in attitude, dress, decorum, and interaction with patients, clients and co-workers. Adheres to the AdvantageCare Physicians' dress code.
•    Demonstrates flexibility in the acceptance and completion of work assignments.
•    Assists with the orientation of new employees participates in the development of other staff members.
•    Ensures that all patients receive personalized, prompt attention and are treated with receptiveness, dignity and respect.
•    May be assigned, on a temporary and short term basis, to other medical offices based on AdvantageCare Physicians' staffing needs.
•    Recognizes and performs duties which need to be performed although not directly assigned. All other duties as assigned.
•    Demonstrates good judgment, discretion and ability to problem solve. Monitors completion of all tasks assigned to LPN/CTA II and CTA I.
•    Participates in the completion of Healthcare Effectiveness Data Information Set (HEDIS)/ Hierarchical Condition Category (HCC) outreach and other AdvantageCare Physicians' initiatives as needed.
•    Attends all meetings as directed by practice administrator/designee.
•    Performs other job-related duties as required

Qualifications:

•    High School Diploma or General Education Diploma (GED) required
•    Formal training from an accredited school with current, unencumbered, NYS license as a Registered Nurse required
•    Current Basic Life Support (BLS) Certification required
•    0-2 years’ related experience required
•    Demonstrated communication and interpersonal skills required
•    Excellent clinical skills required
•    Demonstrated ability to work independently required
•    Demonstrated basic computer skills including typing required
•    Bilingual English/Spanish preferred

This compensation range is specific and considers factors such as (but not limited to) the scope and responsibilities of the position, the candidate’s work experience, education/training, internal peer equity, and market and business considerations. It is not typical for an individual to be hired at or near the top of the range, as compensation decisions depend on each case’s facts and circumstances. Union roles covered by a collective bargaining agreement will compensate in accordance with the union contract.
