Role Description

60%

Contractual Obligations

1) Responsible for conducting daily community-based outreach citywide throughout all (5) NYC boroughs according to the implementation plan for outreach services as projected by program contracts.

A. Conduct programmatic outreach activities (e.g. street outreach, shelters, piers, day centers, parks, health fairs, soup kitchens, house balls and other venues where LGBTQ youth are known to gather).

B. Ensure to create a visible HMI presence in outer boroughs with a focused on reaching youth on the street/subway/parks/near shelters etc. while being accompanied on outreach events by youth outreach interns and occasionally outreach manager.

C. Provide HMI Intake process information and referrals to emergency shelters and other services

D. Develop relationships with outside community contacts (e.g. other homeless youth serving organizations, shelters, governmental agencies etc.)

E. Ensure the usage of a trauma-informed approach to Street Outreach to LGBTQ youth experiencing homelessness

F. Contracted to meet deliverable obligations to perform street outreach 3x per week / 12x per month / 144x per year reaching 1,700 youth engagements

2) Support in the operation of HMI's Drop-In Program

  1. Use existing HMI services such as job readiness, educational initiatives, prevention and intervention programming that addresses domestic violence, sexual abuse and/or exploitation and support group services to help meet the presenting psychosocial issues youth are facing.

B. Assist in program design, recruitment and supervision of young people participating in the program and services.

C. Prepare and coordinate the daily activities/workshops that will be offered during the services for homeless youth's weekly drop-in group.

D. Perform task required during Drop-In program including: signing youth into the program, providing snacks/meals and apprising all youth of activities and services available to them.

E. Staff the drop-in group according to the work plan.

Programmatic Direct Service

Implement services for homeless/marginally housed youth, specifically:

1) Create a warm and welcoming space for homeless and/or street involve youth to participate in programming.

2) Assist youth in assessing On-site Pantry services.

A. General maintenance and replenishment of pantry items including clothing, hygiene items and daily upkeep.

B. Provide quality control with all donations to the pantry including inventory

3) Provide referrals to individual counseling sessions to homeless LGBTQ youth engaged in the program.

4) Work closely to support the case management team to ensure that youth are linked to case management services

5) Facilitate a weekly harm reduction group for street-involved homeless youth and support with street based outreach initiatives as needed.

6) Support case management team with creating and managing individualized housing service plans for youth engaged in the program.

7) Work closely with other HMI programs providing services to the young people; communicate with staff on individual cases; and assist in case planning.

A. Triage youth presenting with challenging issues to the Case Manager or Program's administrative staff for support.

Administrative Duties

1) Track youth member enrollment/activities and contractual requirements and prepare reports and paperwork required for a grants or agency

A. Develop and implement program evaluation and client outcome measures

B. Maintain client level data including progress notes and referral logs

C. Maintain group level documentation including signing sheets and session summaries


Knowledge, Skills and Abilities Required:

  1. Demonstrated experience working with homeless and marginally housed LGBTQ youth.
  2. Demonstrated experience working with NYC housing and shelter systems.
  3. Demonstrated comfort with self-motivated, field-oriented direct practice with clients.
  4. Experience with diverse population of adolescents in the urban setting, including knowledge of lesbian gay, bisexual, transgender and questioning youth from various cultures and
  5. Must be able to work flexible schedule including 12 to 8PM shifts (later on some occasions) Tuesday through Saturday.
  6. Genral knowledge of compliance and regulatory requirements.
  7. Strong knowledge of LGBTQ issues.
  8. Experience working with homeless and/or street-involved youth.
  9. Experience facilitating group discussions and managing a variety of small and large group dynamics.
  10. Evidence of having worked with youth towards testing and engaging in HIV related services.
  11. Experience working in an after-school or drop-in program model.
  12. Excellent verbal and written communication skills, including conflict mediation.
  13. Ability to exercise independent judgment when appropriate strong decision-making skills.
  14. Computer Skills: Strong with proficiency in Microsoft Word, Excel, PowerPoint, Outlook, Canva and Zoom.

Additional Desirable Qualifications:

  1. Bachelors degree in social work, public health, psychology or related human services field.
  2. Bi-lingual (Spanish-English) skills.
  3. Proficiency with Adobe Photoshop and other design editing programs.
  4. Evidence of having worked with youth towards testing and engaging in HIV related services.

Working Conditions:

  1. Work primarily in 2 Astor Place, NYC, except for Outreach work
  2. Traditional program work schedule: Tuesday - Saturday, 12:00 pm to 8:00 p.m.
  3. NOTE: There will be little to no remote work for this position
  4. This position is covered by the 1199 Union
