Job Description
Client Summary:

Summary of Need:
  • Screens all new admitted patients on mother baby meeting a high-risk assessment tool.
  • Assesses patients need for service & documents findings in the medical record
  • Completes a psychosocial assessment on all patients at risk & develops a treatment plan.
  • Provides services including supportive, educational, individual, family & group counseling, planning for the care of the infant, & referral of services.
  • Refers patients that are at risk of HIV infection to the HIV testing & counseling program & provides services for all patients confirmed as HIV+, documenting oin the required case management form, & the follow ups on all HIV+ patients.
  • Provides crisis intervention to individual & groups as indicated.
  • Provides back up social work coverage in the Pediatric Emergency Room & Prenatal Clinic; may provide back up to other areas as needed.
  • Provides services as assigned in related areas in the absence of Social Work staff.
  • Covers neonatal, NICU, labor & delivery, & high risk in distressed patients.


Qualifications/Requirements:
  • Licensed Master Social Worker
  • Valid Licensed Master of Social Work (LMSW) license issued by NYSED, Primary Source Verification
  • Minimum education required: Masters in Social work.

Knowledge, Skills, Abilities:
  • 1 year LMSW experience.
  • Social Worker License
  • Community resources, Client, Client, other community based agency support in order to ensure appropriate linkage of community based services for high risk pregnant women, children, & family members.
  • ICARE values

Shift/Schedule:
  • 10:00 AM-6:00 PM
  • 5 days per week

Contract Length:
  • 13 weeks
